Release of Liability I acknowledge that participation in this competition may cause injuries and/or death.
I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against Squid Industries, the West Coast Flipping Championship, and the persons in charge.
I understand participation in balisong flipping and other activities at this event can be dangerous, Squid Industries requires that all participants (and their adult parent(s) or guardians) to assume all risks associated with balisong flipping and all other activities in the West Coast Flipping competiton signing this general release.
For and in consideration of myself being permitted to participate in West Coast Flipping Championship activities, I hereby voluntarily release, discharge, waive, and relinquish any and all claims or actions for damages for personal injury, permanent disability, death, or property damage which I may have, or which may hereafter accrue to me, as a result of my participation in balisong flipping and event activities during play and while I am at the facility while others play or for any other reason.
This release is intended to discharge, in advance, Squid Industries, West Coast Flipping Championship, it’s officers, employees and agents, and the owners and maintainers of any facility used for balisong/West Coast Flipping Championship practice or activities, from any and all liability arising out of or connected in any way with my child’s participation in the competitions, even though that liability may arise out of negligence or carelessness on the part of Squid Industries, West Coast Flipping Championship, its officers, agents or employees, or the owners or maintainers of any facility used by balisong flipping/West Coast Flipping Championship practice or activities.
I further understand that serious accidents occasionally occur during balisong flipping activities, and that participants occasionally sustain serious personal injuries, death, or property damage as a consequence thereof. Knowing the risks,
I have voluntarily applied for myself to participate in the activity and thereby agree to assume those risks to release and hold harmless Squid Industries and West Coast Flipping Championship, its officers, employees or agents, or the owners or maintainers of any facility used by the Squid Industries and West Coast Flipping Championship for balisong flipping and all event-related activities, who (through negligence or carelessness) might otherwise be liable to me or to my child (or my heirs or assigns) for damages.
I further understand and agree that this release, discharge, waiver, and assumption of risk is to be binding on my heirs, executors, administrators, and assigns.
I further agree to indemnify and to hold harmless Squid Industries and West Coast Flipping Championship, its officers, employees, and agents, or the owners or maintainers of any facility used by Squid Industries and West Coast Flipping Championship for balisong flipping and all event activities, for any loss, liability, damage, cost, or expense which may be incurred as a result of any injury or property damage I may sustain while participating in the activity.
I agree to comply with the program’s stated and customary terms and conditions for participation according to the West Coast Flipping Championship. If I observe any significant change with regard to my readiness for participation in the program, I will remove myself from the program immediately. I have read this Informed Consent/General Release, fully understanding its terms, that I give up substantial rights by signing it, and sign it voluntarily.
Photography and Video Release/Disclaimer:I understand that by participating in this event, I hereby authorize Squid Industries, West Coast Flipping Championship, and Blade Show West to use my likeness and/or the likeness and/or the minority members of my party/ family in a photograph or video in any and all of its publications, including but not limited to all Squid Industries, West Coast Flipping Championship, and Blade Show West printed, digital, and social media publications.
I understand and agree that any photograph or video using my likeness and/or the minority members of my party/ family will become the property of Squid Industries, West Coast Flipping Championship, and Blade Show West.
I acknowledge that since my participation with West Coast Flipping Championship is voluntary, I will receive no financial compensation.
I hereby irrevocably authorize Squid Industries, West Coast Flipping Championship, and Blade Show West to edit, alter, copy, exhibit, or distribute these photos and videos for purposes of publicizing Squid Industries, West Coast Flipping Championship, and Blade Show West programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written and electronic copy, wherein my likeness appears.
Additionally, I waive any right to royalties or any other compensation arising or related to the use of the photographs and videos.
I understand that my participation is AT their OWN RISK, and hereby hold harmless and release and forever discharge Squid Industries, West Coast Flipping Championship, and Blade Show West from all claims, demands, and causes of action, which I, my heirs, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
COVID-19 Liability and Release: I am aware of the existence of the risk of my physical appearance to the venue and my participation in the activity of the Organization that may cause injury or illness such as, but not limited to, influenza, MRSA, or COVID-19 that may lead to paralysis, other serious health issues, or death.
I am fully and personally responsible for my own safety and actions while and during may participation and I recognize that I may be in any case be at risk of contracting COVID-19. With full knowledge of the risks involved, I hereby release, waive, discharge the Squid Industries and West Coast Flipping Championship, the Organization, its board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me related to COVID-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to COVID-19. I agree to indemnify, defend, and hold harmless Squid Industries, West Coast Flipping Championship, the Organization, and all its persons from and against any and all costs, expenses, damages, lawsuits, and/or liabilities or claims arising whether directly or indirectly from or related to any and all claims made by or against any of the released party due to injury, loss, or death from or related to COVID-19.
In the case of an accident or injury, I authorize the staff of Squid Industries, West Coast Flipping Championship, and Blade Show West to make legal decisions, medical decisions, or other decisions concerning me during the West Coast Flipping Championship. I also agree to immediately notify West Coast Flipping Championship, in writing, of any changes to the information above, medical or otherwise. I hereby release Squid Industries, West Coast Flipping Championship, and its sponsors of any legal and financial responsibilities and liability in the event of an accident or injury.
I certify that I have read, fully understand, and accept all terms of this form. I certify that all the information I provided is true. By completing this form, I acknowledge that I have read the foregoing registration form, Liability Release Waiver, and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; That I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in signing this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.
